CCC Living is a searchable database of Rochester-based, state-wide and national community resources. The UR Medicine Complex Care Center created this site to help patients, caregivers and service providers find and use services, programs, and events specific to the needs of each individual.

Filters were created to help you identify what you need by:

Category – education, recreation, support groups

Conditions – autism, sickle cell, cystic fibrosis

Types – event, program, online

We invite you to sign up, search for, or add items to our database. The more people that use it, the better it will become!


The UR Medicine Complex Care Center is a primary care office for adults over age 19 with childhood onset conditions (such as cystic fibrosis, developmental disabilities, muscular dystrophy, sickle cell disease).  We endeavor to make your health the top priority. Your doctor, nurse and/or care manager will work with you and the important people in your life to develop a plan of care that is best for you. We will help you learn about your health condition and how to treat any other diseases or illnesses you may have (e.g. diabetes, hypertension). The more you know, the better decisions you can make about your own health.
